Assessment Process
· On initial inquiry we will discuss your preferred days/times of service and duration of visits. We will take some personal details and arrange for one of our senior staff to visit to carry out the initial assessment (we encourage a friend or relative to be present).
· On the assessment visit a full support plan will be devised with you. The process will identify your needs about how and when you can be supported to live your life, with whom and what this would involve, including personal, practical, dietary and social needs, personal preferences and aspirations.
· The contract and terms of business will be discussed. The assessor will carry out a full risk assessment of the environment, person and any equipment/ materials to be used so that we can reduce the risk of injury to both you and staff.
· You/ your representative will be asked to agree and sign the support plan and contract so that services can commence. You will be left with a copy of both documents.
· The service will be reviewed after 4 weeks and 12 monthly thereafter. A review may be requested at anytime if needs change. Our staff are trained to report any significant changes so that we can always provide the service that best suits your needs.
